Aesthetic and Longevity Treatments in Bali vs the UK: A 2026 Comparison for British Patients
Updated: Sep 29

British patients pay some of the highest prices in the world for aesthetic medicine, and among the highest for private longevity medicine.
At SŌMA Aesthetics and Longevity Club in Uluwatu, Bali, the same manufacturer products cost 35 to 90 per cent less than published London prices, with Profhilo the exception at 16 to 31 per cent; every injectable is administered by a doctor, and longevity diagnostics and 3-to-12-month programmes are available at a fraction of UK clinic fees.
This article compares the two markets at September 2026 prices, explains the standards and regulation involved, and sets out when to do aesthetic or longevity treatments while on holiday in Bali and when not to.
The UK market in 2026: well regulated, expensive, and split by who injects
The UK regulatory framework, under the General Medical Council, the Care Quality Commission and the Joint Council for Cosmetic Practitioners, sets some of the most rigorous practitioner standards in Europe, and the licensing scheme for non-surgical cosmetic procedures being introduced under the Health and Care Act 2022 will tighten it further.
Prices have tightened with it. Anti-wrinkle injections cost £200 to £400 per area in London, with Harley Street clinics at the upper end; a 1 ml syringe of Juvéderm runs £400 to £700; Profhilo £450 to £550 per session. Annual aesthetic maintenance for a typical patient sits at £3,000 to £6,000.
Two structural features shape the experience. Much UK injecting is performed by registered nurses, pharmacist prescribers and dental practitioners, with physician involvement that varies from clinic to clinic; and central London property costs and practitioner salaries mean a quality clinic needs to charge £400 or more per area to be sustainable.
Longevity medicine, which combines biological-age testing, hormone optimisation, NAD+ protocols, peptide therapy and structured year-long programmes, exists in the UK only at the top of the market: programmes at established London clinics are published at £15,000 to £40,000 a year, and a comprehensive longevity blood panel alone at £600 to £1,500.
None of this is unreasonable given the cost base. It is simply not the only option available to a patient who already travels.
How SŌMA is organised, and the standards that apply
SŌMA Aesthetics and Longevity Club is a licensed specialist medical clinic (Klinik Utama, licence 10092501114890005) in Pecatu, Uluwatu, on Bali's Bukit peninsula.
It is regulated by the Indonesian Ministry of Health as a medical facility, not as a spa, and every patient begins with a medical consultation and health screening before any treatment is offered.
The medical director, Dr Shirley Yuliana Kwee, qualified at the University of Glasgow (MBChB) and holds a Graduate Diploma in Medicine from the National University of Singapore. She practised in the UK and Singapore for more than twenty years before founding the clinic, and is a member of the American Academy of Anti-Aging Medicine (A4M).
The clinical team includes two board-certified dermatology, venereology and aesthetics specialists, Dr Boy Ardi Rohanda, Sp.D.V.E, and Dr Ary Wulandari, Sp.D.V.E, together with Dr Muhammad Nadim, M.Biomed (regenerative medicine), Dr Andrew Uidjaja, M.Biomed (anti-ageing and aesthetic medicine) and Dr Revaniar Poetri Calvan (dermatology, venereology and aesthetics).
Three policies matter most to international patients.
Injectables are administered only by doctors. That matches the practice of medically led UK clinics and exceeds the standard at many high-street injection clinics. Every product is BPOM-registered (BPOM is Indonesia's medicines regulator) and bought through the manufacturer's authorised Indonesian distributor, with the batch number recorded in the patient file and the sealed vial shown before it is opened. And prices are published in full, in rupiah, on the pricing page, with the total including 11% tax quoted in writing before treatment.
Two developments in 2026 are relevant to this comparison. In August, Dr Nadim completed the Cynosure Lutronic Academy certification for XERF, the dual-frequency monopolar radiofrequency skin-tightening platform, making SŌMA among the first clinics in Bali to offer it. In September, the clinic announced a research and knowledge collaboration with the Oxford Biohacking Society, a University of Oxford society working across biotechnology, neuroscience and longevity; SŌMA Aesthetics and Longevity Club physicians are accredited by The American Academy of Anti-Aging Medicine (A4M) and ISSCA.
Price comparison, September 2026
UK ranges are drawn from published pricing pages of Harley Street and central London clinics and from industry benchmarks compiled in April 2026. SŌMA prices are the clinic's current published rates, converted at the 21 September 2026 rate of IDR 23,803 to the pound and shown before 11% Indonesian tax.
Treatment | UK, London (published range) | SŌMA, Uluwatu (Sept 2026) | Saving |
Botox, single area (about 20 units, Allergan) | £200–300 | £84 | 58–72% |
Botox, upper face, three areas (about 40 units) | £350–500 | £168 | 52–66% |
Hyaluronic acid filler, per ml (Juvéderm) | £400–600 | £252 | 37–58% |
Profhilo, 2 ml session | £480–550 | £378 | 20–31% |
Rejuran Healer, 2 ml session | £550–750 | £273 | 50–64% |
Comprehensive longevity bloodwork | £800–1,500 | £357 | 55–76% |
Private dermatology consultation | £200–400 | £32 | 84–92% |
12-month longevity programme | £15,000–40,000 | £5,251 | 65–87% |
Two observations follow from the table. Profhilo is the one product on which the gap has narrowed, because SŌMA prices it per 2 ml session at a rate close to the lower end of the London range; the saving is real but more modest compared to other treatments, and a patient choosing Bali for Profhilo alone would be choosing it for the doctor-administered protocol rather than the price. At the other end, longevity diagnostics, dermatology consultations and full programmes are 55 to 90 per cent cheaper, which is where the trip earns its fare.
What has been added to the SŌMA menu in 2026
The comparison table above concentrates on treatments with a published benchmark in the home market. Several of the clinic's newer services have no like-for-like equivalent, or are priced so differently abroad that a percentage comparison would mislead, and are listed here with their SŌMA price instead.
XERF skin tightening. Cynosure Lutronic's monopolar radiofrequency platform delivers 6.78 MHz and 2 MHz together to heat the deep dermis and the SMAS layer, with cryogen cooling on each pulse and no anaesthetic. A session takes 45 to 60 minutes and has no downtime. IDR 6,000,000 (£252) for the forehead, eye area or double chin, IDR 31,500,000 (£1,323) for the full face and IDR 40,000,000 (£1,680) for face and neck.
Juläine. Nordberg Medical's Swedish poly-L-lactic acid collagen biostimulator launched in Indonesia on 29 August 2026, and SŌMA is the first clinic in Bali to offer it, alongside Sculptra, Radiesse and Juvelook on the collagen biostimulators menu. IDR 25,000,000 (£1,050) per vial.
Body composition analysis on the seca mBCA Ultra. A medical-grade 8-point bioelectrical impedance analyser, validated against MRI and the four-compartment model, reporting fat mass, segmental skeletal muscle, visceral fat, body water and phase angle in 24 seconds. It is used at baseline and every four weeks in the GLP-1 weight-management programme, and quarterly in the longevity programmes. SŌMA is the first clinic in Bali with the device.
Regenerative treatments. PRP, CGF and PRF, APAG from the patient's own blood (PRP facial injection IDR 3,500,000, £147; PRF natural filler from IDR 4,500,000, £189), exosome therapy from a GMP laboratory (from IDR 7,500,000, £315) or autologous, derived from the patient's own blood (from IDR 12,000,000, £504), secretome injections for injury recovery, and stem cell therapy following medical screening. SŌMA is the only clinic in Bali offering autologous exosomes. Exclusive to SŌMA, autologous topical cream prepared from patients' own plasma and CGF in the UK is priced at £900; at SŌMA, stand-alone cream preparation is priced at IDR 2,500,000 (£105), and as an add-on to PRP or PRF treatment is priced at IDR 1,500,000 (£63).
Signature programmes. Multi-day, physician-designed programmes that begin with a complimentary pre-arrival teleconsultation: REJUVENATE (skin, body and hair) starting from IDR 7,000,000 (£294), RECHARGE (energy, sleep and recovery) startign from 8,000,000 (£336), REBUILD (pain, mobility and strength) startign from 5,000,000 (£210), OPTIMIZE (hormones, metabolism and body composition) startign from 5,000,000 (£210), FUTURE-PROOF (biological age, genomic risk and regenerative protocols) startign from 30,000,000 (£1262), RADIANCE for weddings and events startign from 6,000,000 (£252), and the weekend RESET for guests already in Uluwatu.
Longevity programmes compared
SŌMA's longevity medicine is built around measurement first: comprehensive bloodwork (IDR 8,500,000, £357), DNA-methylation biological-age testing (IDR 6,500,000, £273), body composition on the seca mBCA Ultra, and hormone and metabolic review, before any intervention is prescribed. Interventions such as NAD+ infusions, peptides, hormone optimisation and regenerative injections are added only where they are appropriate for the individual, and the level of evidence behind each is explained.
Programme | Duration | SŌMA price (IDR) | Approx. GBP |
Foundation Longevity Program | 3 months | 38,000,000 | £1,596 |
Optimization Longevity Program | 6 months | 68,000,000 | £2,857 |
Executive Longevity Program | 12 months | 125,000,000 | £5,251 |
Published UK equivalents at established London longevity clinics run £15,000 to £40,000 for twelve months. Programme fees at SŌMA exclude 11% tax; what each tier includes is confirmed in writing after the initial longevity consultation (IDR 1,500,000, £63). Between visits, GLP-1 prescriptions, peptide protocols and hormone therapy are followed up by video consultation, with a quarterly progress review and annual re-assessment bloodwork.
Travel and timing from the UK
From London Heathrow to Denpasar, expect 16 to 19 hours with one stop, routing through Doha, Dubai, Singapore or Bangkok. Return economy fares run £600 to £1,200, premium economy £1,500 to £2,800 and business class £3,500 to £6,500. Bali is seven hours ahead of British Summer Time and eight hours ahead of GMT.
An eight-day visit is a sensible minimum: two days to arrive and adjust, a consultation with examination, blood draws and photography on day three, treatment once results return on days four and five, follow-up treatment and review with take-home prescriptions on days six and seven, and departure on day eight. A longevity programme is better served by 10 to 14 days, so that comprehensive bloodwork, biological-age testing and body composition can all be completed and the first protocol delivered before departure. Ongoing GLP-1, peptide or hormone protocols are then continued from the UK with quarterly video reviews and an annual return visit for re-testing.
Who the Bali option suits, and who it does not
The trip suits patients who already travel long-haul comfortably, who want full longevity diagnostics and a programme rather than an aesthetic top-up, who are paying £4,000 or more a year for UK maintenance and would rather consolidate it, or who want treatments the UK market either does not offer or prices at a premium: autologous exosomes, XERF, Juläine, structured 12-month programmes. It suits anyone content to combine the medical work with a substantial holiday.
It does not suit a single-area Botox top-up between meetings, treatments that need frequent local follow-up such as a laser hair removal course, an active medical condition that requires continuous management by a local doctor, or a patient who is uncomfortable with long-haul travel for medical reasons.
Frequently asked questions from British patients
How does Indonesian regulation compare with the MHRA?
BPOM, Indonesia's medicines regulator, licenses the import of aesthetic injectables and prescription medicines, including Botox, hyaluronic acid fillers, Profhilo, Rejuran, Sculptra, Radiesse and GLP-1 medicines. The same manufacturers, Allergan, IBSA, PharmaResearch, Galderma, Merz and Novo Nordisk, supply Indonesia through authorised distributors. The product is identical to the UK product; the regulatory pathway and the cost of care differ. SŌMA buys only through authorised distributors and records batch numbers in each patient's file.
Is there continuity of care after returning to the UK?
SŌMA provides WhatsApp and video follow-up for international patients with no time limit. Complications from injectables needing in-person care are rare and almost always present within the first few days, usually within the travel window. For continuing medication, the clinic's documentation can be shared with a GP or private practitioner in the UK, or prescriptions renewed at an annual return visit.
What if the result is not what the patient wanted?
Hyaluronic acid fillers can be dissolved with hyaluronidase, which is available at SŌMA as in UK clinics. Anti-wrinkle injections wear off over three to five months. Biostimulators such as Profhilo, Sculptra, Juvelook and Juläine build gradually and cannot be reversed, which is why the clinic's protocols are deliberately conservative: a smaller first treatment, review at four to six weeks and more only if needed.
Can recent NHS or private bloodwork be used instead of repeating tests?
Yes. Results shared before the visit are reviewed by the doctor, who will recommend only the additional markers required for the protocol under consideration. There is no requirement to repeat tests recently completed.
How can a British patient verify SŌMA before travelling?
Check the Klinik Utama licence (10092501114890005), confirm Dr Shirley Yuliana Kwee's University of Glasgow MBChB, read the team credentials on the About page, read recent TripAdvisor and Google reviews, request batch documentation for any product, and book a pre-travel video consultation.
Sources and method
UK benchmark ranges were compiled in April 2026 from published 2026 pricing pages of Harley Street and central London clinics, the Harley Academy injectable-pricing benchmark and published programme tiers at London longevity and preventive-medicine clinics. SŌMA prices are the clinic's published September 2026 rates in Indonesian rupiah, exclusive of 11% government tax, converted at mid-market exchange rates on 21 September 2026 (USD 17,790; EUR 20,418; GBP 23,803 per unit). Benchmark ranges should be verified against current published prices before any clinical or travel decision. This article is educational and does not constitute individual medical advice.
